Request Medication

To request medication already on repeat prescription, please use the NHS App or Online Services (register for a username and password via reception).

For all other medication requests you may use the form below. It’s helpful to include why you want the medication if you’ve not had it in a while. Please include the chemist you’d like the prescription to be sent to if you’ve not already set this with us.

If you can’t use computers you may use the surgery postbox for requesting.

By using this form you will be sending information about yourself across the Internet. Whilst every effort is made to keep this information secure, you should be aware that we cannot offer any guarantees of absolute privacy.

Furthermore, this service is in place for your convenience, and works >99% of the time. However it does rely on technology that rarely fails, and therefore there is a very small risk your request is missed. We will always approve or query your request within 3 working days (usually within 1) so if the chemist says there is no script after this time please contact us.

Also, by sending this form you are indicating your agreement that the surgery may contact you to discuss the information contained in this form.
