How to register at Cornerways Surgery

New patient registration

Patients may register at Cornerways Surgery if they reside within the practice area. The practice area is contained between the sea and the railway line. The easterly boundary is west side of Downview Road (odd numbers) and the westerly boundary is east side of Shaftesbury Avenue. Patients moving out of this area will usually have to register with another surgery.

Please collect the GMS1 registration form and questionnaire from the surgery. Fully complete and return these forms to the surgery during opening hours for checking by the receptionist together with two forms of identification – one photographic and one showing up to date address of the property within our practice boundary.

After registering the forms onto the computer system, you will be contacted by the surgery to make a registration appointment with a clinician at the surgery.  This appointment may take up to two weeks to take place.

Please ensure you have sufficient medication etc. prior to leaving your previous surgery.
