Dear Patients,
We have been developing plans over the last few months to improve the way we monitor long-term health conditions and both conditions and medications that require blood monitoring. From April 2022 we will introduce our month of birth recall system. For example, if we think you need a blood test once per year and you are born in July, then we’ll contact you in June to organise you blood test. If we think you need a test twice a year, we’ll contact you again 6 months after that. For some conditions like dementia, rheumatoid arthritis, severe frailty and severe mental illness, we’re introducing an annual GP review appointment face to face. Where you’re housebound we’ll organise a visit. For most patients, you’ll be invited by text message to telephone the surgery to book your test, so please make sure we have your up to date mobile phone number. For some patients we know we’ll need to call you to organise. We hope such preventative and holistic ‘annual reviews’ will be useful.
